National Transportation Safety Board findings: Manufacturer or Builder error. Aircraft unable to sustain flight due to inability of engine to maintain proper RPM's. Cause: Nut on throttle linkage not properly tighten and/or lack of "lock tight" applied. Damage moderate, all 4 blades of the prop destroyed, both landing gears torn off, several large holes on wings, flaps, fuselage behind tail wheel blown off. Several small holes on fuselage and wings and flaps. With professional help this aircraft could fly again.
